Important things to know about Ashland County
- Two Ashland-specific intake forms are mandatory: the Case Designation Sheet and the DRH Form.

- Parenting Proceeding Affidavit (Aff. 3) — 5-year address history for each child. Required in every case with kids.
- Health Insurance Affidavit (Aff. 4) — Documents each parent's ability to insure the children.
- Shared Parenting Plan (Form 20) — Use this if you'll share legal custody. Both parents must sign.
- Parenting Plan (Form 21) — sole custody — Use if one parent will be the residential parent. Pair with Parenting Judgment Entry.
- Parenting Judgment Entry (Form 22) — Sole-custody order naming the residential parent.
- Personal Identifier Form (one per child) — Separate sheet for each minor child's sensitive info.
- ODJFS IV-D Child Support Application — Email directly to hadkins@ashlandcommonpleas.com — do NOT file with Clerk.
- Ohio Child Support Worksheet — Use the Ohio Child Support Calculator and attach the printed worksheet.
