How to File Adoption in Fairfield County

2. File the petition with supporting forms. File Form 18.0 Petition for Adoption of Minor with the Contact Information Form (75.3(A)), the Self-Representation Acknowledgment (75.1) if you are pro se, and any biological-father/consent forms (e.g., 103.8). Court costs are $300.
3. Handle consent. Obtain Consent to Adoption (18.3) from each person whose consent is required, or ask the court to find consent is not required (18.4) under R.C. 3107.06–.07. Complete the Putative Father Registry check (JFS 01695).
