Filing for Dissolution in Lake County, Ohio
Lake County's Domestic Relations Court is in Painesville with a single judge (Hon. Colleen A. Falkowski) and one quirk that trips up almost every pro se filer: the Clerk of Courts is in a DIFFERENT building from the court — you file and pay at 25 North Park Place (basement of the Courthouse Annex), not at the court at 47 North Park Place. A tunnel on the lower level of the main courthouse connects to the Clerk's office. Two things make Lake County easier than most: (1) the court provides complete dissolution packets WITH a built-in checklist (one for with-children, one for without) so you can verify nothing is missing before filing; and (2) Lake County has two separate parenting time schedules — Local Rule 23A for parents living close to each other and Local Rule 23B for long-distance parents — more detailed than most counties. Filing fees are $306 without children, $326 with. Spanish-language resources are available for the parenting class brochure and the indigency application.

- Case Designation Sheet — Tells the court what type of case you are filing. Required with every new case.
- Petition for Dissolution of Marriage and Waiver of Service (Form 17) — The main joint petition — both spouses sign together.
- Separation Agreement (Form 19) — Spells out exactly how you've agreed to divide property, debts, spousal support, and all other terms. Both spouses sign.
- Waiver of Service of Summons — Because you're filing jointly, both spouses waive formal service.
